Objectives:
From the results of the Needs
Assessment, the Delaware Nation -
Division on Aging Nutrition Program
determined that the following
objectives would be most effective
to meet the needs of the native
population: nutrition services,
transportation, chore services,
information, assistance and family
support.
In response to the identified
disparities, the Delaware Nation -
Division on Aging Nutrition program
will implement the following
objectives to meet the needs of the
Native American elderly in the
service area of the tribe. These
objectives are:
Nutrition
Services (Congregate and
Homebound meal delivery).
Nutrition
and Health Education.
Supportive Services.
Native
American Cultural Education
and Preservation
Each
of these objectives is projected to
be an on going program activity,
throughout the three-year project
period. This will enable the
Delaware Nation - Division on Aging
Nutrition Program to properly
address the needs of the Native
American Elder continually with
emphasis on improving the health and
welfare of each participant. The
evaluation of the Delaware Nation -
Division on Aging Nutrition Program
will be conducted in the summer of
2005.
