Program Objectives:

 

From the results of the Needs Assessment, the Delaware Nation - Division on Aging Nutrition Program determined that the following objectives would be most effective to meet the needs of the native population: nutrition services, transportation, chore services, information, assistance and family support.

In response to the identified disparities, the Delaware Nation - Division on Aging Nutrition program will implement the following objectives to meet the needs of the Native American elderly in the service area of the tribe. These objectives are:

  • Nutrition Services (Congregate and Homebound meal delivery).
  • Nutrition and Health Education.
  • Supportive Services.
  • Native American Cultural Education and Preservation

Each of these objectives is projected to be an on going program activity, throughout the three-year project period. This will enable the Delaware Nation - Division on Aging Nutrition Program to properly address the needs of the Native American Elder continually with emphasis on improving the health and welfare of each participant. The evaluation of the Delaware Nation - Division on Aging Nutrition Program will be conducted in the winter of 2010.


 

Delaware Nation Special Diabetes Program for Indians (SDPI)

 

All the following criteria must be met in order to receive assistance:
(proof of each is required)

 

  • Delaware tribal member or non-indigenous Native American
  • Reside within our service area
  • Do not already benefit from another tribal SDPI
  • Have been diagnosed with diabetes mellitus

 

Please fill out the application completely and sign and date it. A separate application for each must be filled out and approved for further assistance. Each application is provided separetly below.
 

 

"Prescription from Doctor" means that you must either:

  • Submit a "Health Summary" from the IHS facility you use showing your diabetes mellitus or
  • Have you primary care physician write on a prescription pad stating that you have been diagnosed with diabetes mellitus and that he/she recommends you have good walking shoes, dentures, and/or eyeglasses.
